Corrective action is not about fixing mistakes.

It is about eliminating the cause.

If the same issue reappears, the root cause was not addressed.

In business, this is called root cause analysis.

But the principle applies everywhere:

Repeated issues signal systemic weakness.

Whether in an organization or elsewhere, patterns repeat until causes are resolved.


Strong systems solve causes — not symptoms.
